New-patient registration forms
To make your first visit efficient, we'll send your intake forms ahead of time so you can fill them out at home instead of in the waiting room.
Booking online? We’ll follow up by email with everything you need: medical and dental history, contact information, and consent forms. You can complete it at your own pace before you arrive.
Prefer to do it in person, or booked by phone? Just arrive about fifteen minutes early and our front desk will help you through it. It's no different from filling it out at home, just a little less convenient.
Call the office at (504) 895-3400 if your forms haven’t arrived a couple of days before your visit, and we’ll send them right over.

- Any recent X-rays, if you have them
